Welcome to OR-TEC's new Project Information Forms page. Please click on the link below to the piece of equipment most suited to your application. (If you are not sure which piece of equipment you need please click Contact us and send a general information request email)
When you have clicked on the link you want a PDF format document will open. Then do the following:

• Save the form to your computer
• Fill out the form (Use Tab key to move easily between fields)
• Email form to OR-TEC (info@or-tec.com)
• Re-Save form to your computer before closing to ensure you keep copy for yourself
• To Re-Use form at a later date simply re-open form from your computer, Save As a new name and click Reset

Please feel free to distribute forms to your staff, co-workers etc. Some of the forms are not interactive at this point. These can be printed and filled out the old-fashioned way with a pen and faxed to OR-TEC at 440-232-4224
